Welcome to the Transform Portal! This page is for those who have completed Transform and attended a Transform retreat. This is the place for you to submit your application to become a Network member.
To start the process of becoming a Network Member, you need to submit the following:
- Transform FIC Network Application
- Transform Acknowledgements – Core Values and Statement of Faith forms
Your Hub leader will submit the other form for you once you have completed the network application. The hub leader will submit:
- The Transform Hub Leader’s Full Assessment Form
